1.	CITY OF PHOENIX ORDINANCE G-4396

	Per City of Phoenix Ordinance G-4396, these plans are for official use only and may not be shared
	with others except as required to fulfill the obligations of your contract with the City of Phoenix.

2.	POLE/SIDEWALK CLEARENCE NOTE

	A Minimum 3 Feet Of Clearance Is Required Between All Traffic Signal Poles And Sidewalk Ramps.

3.	CONTRACTOR NOTE FOR CITY OF PHOENIX POLE 
	(Show this note only when contractor is providing special poles)

3.1	The contractor shall provide and install ___ City of Phoenix type _________ pole(s) with
	anchor cage, nuts & bolts, luminaire riser, 8 Luminaire mast arm and ______ ft mast arms as
	per C.O.P. St. Trans> Dept. drawing #7690.

3.2	Signal Poles, Mast Arms, mast Arm Extensions, luminaire extensions, and 8 luminaire mast
	arm shall be power coated. Contact the Traffic Signal Construction Supervisor at (602) 262-6733 to confirm color.

3.3	Separate pull wires shall be installed from each tendon on the Signal Mast Arm and the
	Luminaire Mast Arm to the hand hole at the base of the Traffic Signal Pole.

4.	WORK TO BE PERFORMED BY CONTRACTOR:

	Contractor to provide all underground work including but not limited to:

		All trenching
		All foundations
		All Junction boxes w/ 18 of sump
		All Conduit runs with #10 bond wire installed
		Provide and install the power service pedestal and foundation
		Traffic Signal equipment as indicated in note 3.1

5.	WORK TO BE PERFORMED BY CITY OF PHOENIX CREWS:
	
	City Of Phoenix Traffic Signal Crews Will Provide and Install All Aboveground traffic Signal Equipment including:

		All Poles except as indicated in note 3.1
		All Signal Heads
		All Pedestrian Heads
		Pedestrian Push Buttons
		Controller
		All Luminaries
		All Illuminated Street Signs
		All Signal Wiring
		All Loop Detectors

6.	WARRANTED SIGNALS: 
	(Show this note only on warranted signals)

	All work to performed by City Of Phoenix Traffic Signal Crews.

7.	The Contractor shall notify the City of Phoenix, Traffic Signal Shop (262-6733)
	a minimum of fourteen (14) calendar days prior to beginning any traffic signal work.
